Kathleen M. Rand, Raymond G. Hoffman, Wade Mueller, Mark G. The anti-VEGF antibody, bevacizumab, is standard treatment for patients with recurrent glioblastoma. In this setting, traditional anatomic MRI methods such as post-contrast T1-weighted and T2-weighted imaging are proving unreliable for monitoring response. Here we evaluate the prognostic significance of pre- and posttreatment relative cerebral blood volume rCBV derived from dynamic susceptibility contrast MRI to predict response to bevacizumab. Thirty-six participants with recurrent high-grade gliomas who underwent rCBV imaging 60 days before and 20—60 days after starting bevacizumab treatment were enrolled. Tumor regions of interest ROIs were determined from deltaT1 maps computed from the difference between standardized post and precontrast T1-weighted images. With a With the identification of multiple signaling pathways and growth factors essential for tumor angiogenesis, several new anti-angiogenic drugs have been developed.

A stroke can be dramatic—and devastating. As part of the brain is starved of its blood supply, cells may die. If a large number of brain cells die, with them may go some of a person’s ability to speak, move, and remember. Yet there is also another type of stroke—one that is far subtler and harder to spot. Termed a “silent stroke,” it creates areas of damage in the brain. These areas of dead brain cells are smaller than with a traditional stroke and they impact less-functional areas of the brain, but researchers are finding that they can still have a significant and lasting impact on memory.

John C. Wood; Guidelines for quantifying iron overload. Both primary and secondary iron overload are increasingly prevalent in the United States because of immigration from the Far East, increasing transfusion therapy in sickle cell disease, and improved survivorship of hematologic malignancies. This chapter describes the use of historical data, serological measures, and MRI to estimate somatic iron burden.

With incorporation of creatinine analyses to blood gas and other point-of-care platforms the test is now available outside the laboratory, at the bedside and in the clinic. This is the first of two articles that highlight the clinical value of creatinine measurement in the radiology department. An ever-increasing number of patients submitted for X-ray and other body imaging examinations such as computed tomography CT and magnetic resonance imaging MRI scan are given image-enhancing contrast agents that can be associated with adverse effect.
Among these adverse effects are two conditions that are the focus of the two articles: nephrogenic systemic fibrosis NSF and contrast-induced nephropathy CIN. Consideration of the significance of serum creatinine measurement in CIN is reserved for the second article. Here the focus is NSF and how measurement of serum creatinine is used to identify those patients at risk of the condition.
The article will include discussion of what is known about the pathogenesis of NSF, the triggering role of pre-existing renal disease along with the significance of MRI scanning.

There are many factors your doctor will rely on to form an accurate multiple myeloma diagnosis — overall health, symptoms and diagnostic tests of blood, urine and bone marrow play an important role. Being diagnosed with a disease like multiple myeloma can feel overwhelming. These are some of the steps you can take to begin management of your disease.
Knowing the classification of your disease is very important in deciding when it is appropriate to begin treatment. Classification also plays an important role in determining the stage of multiple myeloma.

To document the signal characteristics of intracerebral hemorrhage ICH at evolving stages on diffusion-weighted images DWI by comparison with conventional MR images. DWI showed that hematomas were hyperintense at the hyperacute and late subacute stages, and hypointense at the acute, early subacute and chronic stages. Invariably, focal hypointensity was observed within a hyperacute hematoma. At the hyperacute, acute and early subacute stages, hyperintense rims that corresponded with edema surrounding the hematoma were present.
The mean ADC ratio was 0. In patients with acute stroke, an understanding of the characteristic features of ICH seen at DWI can be helpful in both the characterization of intracranial hemorrhagic lesions and the differentiation of hemorrhage from ischemia. The usual pattern of magnetic resonance MR appearances corresponding to the different stages of intracerebral hemorrhage ICH is well known. The generalized model for the appearance of ICH on MR images attributes the various signal intensity patterns of evolving ICH to the oxygenation state of hemoglobin and the integrity of the red blood cells 1 , 2.
Despite the frequent use of conventional MR imaging to evaluate the appearance and underlying biophysical basis of evolving ICH over the past few years, diffusion-weighted MR imaging DWI has only recently been recognized as a valuable investigative resource 3 – 8. DWI is a relatively new technique which, by using additional strong diffusion-sensitizing gradients, is extremely sensitive to changes in the microscopic motion of water protons.
It has proved valuable in the study of the natural history of ischemic stroke and is now a promising technique for the early detection of cerebral infarction in routine clinical practice 9 –

In , it is estimated that 26, patients will be diagnosed with a malignant primary brain tumor in the United States, with more than half having the diagnosis of glioblastoma 1. Most patients with glioblastoma undergo computed tomography of the brain Figure 1 upon initial presentation. Many institutions will also capture T2w gradient echo and diffusion weighted sequences.
Maximal safe debulking surgery is typically recommended as the initial standard of care. Neurosurgeons will often utilize high-resolution MRI 0. Standard imaging also can identify other important characteristics of the mass in situ , including the volume of various tumor sub-regions i. The spatial heterogeneity of glioblastoma is macroscopically apparent on standard MRI sequences; beyond the tumor bulk described above, pathology studies demonstrate microscopic tumor infiltration throughout the peritumoral edema, which appears hyperintense on T2-FLAIR sequences Figure 2.
Edema appears to develop in response to angiogenic and vascular permeability factors associated with infiltrating tumor 7 , 8. As tumors outgrow the native blood supply, the resultant ischemia triggers further secretion of angiogenic factors that promote vascular proliferation 9 , A recent meta-analysis of over 40, glioblastoma patients demonstrated that gross-total resection was associated with improved survival as compared to subtotal resection Historically, the determination of gross-total resection was made in the operating room by the neurosurgeon.

Although MRI is often thought of as not being sensitive to acute hemorrhage, this is not, in fact, true particularly with more modern sequences 5,7. The factors that affect the appearance of hemorrhage on MRI vary according to the sequence. The oxygenation state of hemoglobin and the location of either contained within red blood cells or diffused in the extracellular space have a tremendous effect on the imaging effects of blood. The three hemoglobin states to be considered are oxyhemoglobin, deoxyhemoglobin and methemoglobin.
Additionally, blood products that may be subtle on conventional MRI MRI and FLAIR, the DWI is particularly helpful in dating MS plaques.
Cerebral hypoxia occurs when there is not enough oxygen getting to the brain. The brain needs a constant supply of oxygen and nutrients to function. Cerebral hypoxia affects the largest parts of the brain, called the cerebral hemispheres. However, the term is often used to refer to a lack of oxygen supply to the entire brain. Brain cells are very sensitive to a lack of oxygen. Some brain cells start dying less than 5 minutes after their oxygen supply disappears.
As a result, brain hypoxia can rapidly cause severe brain damage or death.
